Consciousness	2	Fully awake or responds easily to verbal stimuli or return to pre-op level (must be a "1" at the conclusion of monitoring
	1	Responds to tactile stimuli and exhibits presence of protective reflexes
	0	No response to verbal or tactile stimuli

Skin Color	2	Within normal limits
	1	Skin color is pale, dusky, or blotchy
	0	Cyanotic

Modified Ramsey Scale

2	Drowsy, anxiety decreased

ASA Physical Classifications
I	Normal, healthy adult
II	Patient with mild systemic disease
III	Patient with severe systemic disease
IV	Patient with severe systemic disease that is a constant threat to life
V	Moribund patient not expected to survive without surgery
Notes ASA Class I-III may receive deep sedation in the ER ASA Class III-V require prompt anesthesia consult before procedures
